FCHIP

FairBanks Community Health Intelligence Platform

FCHIP is FairBanks' community health intelligence platform. It sits on our Community Reach model and turns information from the field and our facilities into clearer early warnings so care can move from reacting after illness to preventing problems sooner.

FCHIP — FairBanks Community Health Intelligence Platform; community data and climate signals feeding Data & Feedback

FairBanks is more than a medical centre. We connect community members, community health workers (CHWs / VHTs), outreach programmes, our Medical Centre, research partners, and livelihoods support — including CHIS (Community Health Insurance Scheme). FCHIP is the technology layer that helps that whole loop learn and improve.

Your health, our mission.

Why FCHIP matters

Signals that used to arrive too late

In many communities, clinics only see people after they are already sick. Outreach notes stay in paper books or separate systems. Weather and place-based risk rarely join the picture. FCHIP helps FairBanks bring these signals together so teams can spot rising risk earlier.

Climate-linked risk

Fever clusters spotted after heavy rain, before a health post is overwhelmed.

Maternal follow-up

Mothers who need closer follow-up are flagged early, not discovered weeks later.

Community screening

Neighbourhoods where blood pressure and diabetes need more screening stand out sooner.

How FCHIP works

Capture. Analyse. Alert. Act.

01 — Capture

Field & facility data

Offline mobile forms for CHWs/VHTs, secure links to facility systems (EMR/HMS), and climate feeds.

02 — Analyse

Risk scoring

AI-assisted risk scores, GIS clustering, and climate–health fusion.

03 — Alert

Worklists & dashboards

Worklists for CHWs, facility dashboards, and partner / district views.

04 — Act

Outreach & follow-up

Outreach, referrals, stock planning, and follow-up — then learn and improve again.

What FCHIP connects

FCHIP is built to receive structured health information from FairBanks and partner sources along our Community Reach cascade.

  • Community Health Workers and Village Health Teams (home visits, referrals, follow-up)
  • Schools and community outreach programmes
  • FairBanks Medical Centre and partner clinics and hospitals
  • Existing facility record systems, through secure data connections — so clinics don't have to replace software they already use
  • Pharmacies and medicine points, where data is available
  • Maternal and child health, chronic disease, and geriatric (Gericare) programmes
  • Climate and weather feeds, combined with maps (GIS), for climate-aware early detection

Deep technology, rooted in FairBanks

Practical tools, deeper intelligence

AI

AI-assisted risk scoring

Spot rising disease risk, maternal complications, and NCD hotspots earlier — with explainable alerts for field and facility teams.

GIS

Maps that show where risk sits

Visualise cases, climate overlays, and risk zones by village, parish, or facility catchment.

Climate meets community health

Rainfall, heat, and extreme-weather feeds join CHW and clinic signals for climate-aware early detection.

📱

Built for CHWs and VHTs

Offline-capable forms for household visits, vitals, referrals, and follow-up — then sync when connectivity returns.

One view for the Medical Centre

Trends, alerts, completed referrals, and outreach priorities for clinicians and programme managers.

🔒

Connect systems you already use

Authenticated, consent-aware data APIs let existing EMR and hospital systems share clinical data into FCHIP — without ripping out current software.

FCHIP turns community and facility data into earlier action.

CHWs / VHTs

Partners & districts

Shared evidence for planning, early warning, and impact.

What FCHIP helps predict

Six places intelligence changes the outcome

Six FCHIP prediction areas: surveillance, maternal, NCDs, child health, medicine demand, cascade metrics
Surveillance

Disease surveillance

Rising fever reports in neighbouring villages, plus heavy rainfall and a tight map cluster, can flag possible outbreak risk so outreach and testing can move early.

Maternal health

Fewer missed follow-ups

Home-visit signals can highlight higher-risk pregnancies and missed antenatal care so CHWs and clinicians can follow up sooner.

Chronic disease

Targeting NCD screening

Blood pressure and screening data from camps, clinics, and workplaces can show where hypertension and diabetes need targeted action.

Child health

Nutrition & immunisation

Growth, immunisation, and diarrhoea patterns from homes, schools, and clinics can trigger nutrition and immunisation support.

Medicine demand

Planning stock ahead

Disease trends, climate patterns, and use at the facility can help plan stock before shortages.

Cascade metrics

Closing loop gaps

CHW activity, completed referrals, school sessions, and CHIS / livelihood signals help managers close gaps in the Community Reach loop.

Example: fever reports, rainfall, and GIS cluster trigger FCHIP early action
A worked example

Climate-aware early warning

Three neighbouring villages report rising fever through VHTs. FCHIP joins that with two more signals arriving the same week.

Field reportsRising fever reported through VHTs in three neighbouring villages.
Climate feedRecent heavy rainfall recorded across the same area.
GIS clusteringCases sit inside one tight geographic cluster, not scattered at random.
FCHIP flagElevated outbreak risk — FairBanks teams deploy testing, outreach, and pharmacy pre-stocking before caseloads spike.
Where FCHIP sits

The Community Reach loop

Needs flow down through the cascade; care and solutions flow back up. FCHIP powers the Data & Feedback layer that keeps the loop continuous — learn, improve, and serve again.

Our stage

We have a working FCHIP MVP. Our focus now is validating it inside FairBanks' live Community Reach — with CHWs/VHTs, facility teams, GIS and climate signals, and secure links to facility systems — before wider district and regional scale.

Questions

Frequently asked

FCHIP means FairBanks Community Health Intelligence Platform. It is FairBanks' technology component that helps Community Reach and the Medical Centre see health risks earlier using community data, facility signals, maps, and climate information.

No. FairBanks remains a community health ecosystem: Community Reach, the Medical Centre, CHIS, and livelihoods. FCHIP is the Data & Feedback intelligence layer on that model.

CHWs and VHTs in the field, FairBanks Medical Centre teams, programme managers, and — as we scale — district partners, NGOs, and research collaborators.

Structured data from CHWs/VHTs, schools, community programmes, clinics and hospitals, secure facility system feeds where connected, pharmacies where available, and climate / weather APIs — always with privacy and consent rules in mind.

No. FCHIP is designed to expose secure data APIs so existing EMR/HMS systems can share clinical data into the platform without replacing what facilities already use.

A working MVP exists. We are validating it inside FairBanks' live Community Reach cascade before wider district and regional scale-up.

Climate feeds, for example rainfall and heat, are fused with community and facility health signals and GIS maps so teams can act on climate-linked risk earlier — such as fever clusters after heavy rain.
